Objectives: To conduct a systematic review and meta-analysis regarding the efficacy and safety of inhaled corticosteroids for COPD exacerbationsin HIV-positive patients. Methods We performed a search of Cochrane's register of trials using the key terms 'corticosteroids', 'copeldiaphragm', 'cortical fibrosis', (corticostical) and (corticosteroid) in order to identify all trials and eligible studies published between October 1, 2008, and February 2, 2011. Key words and search terms included were (CCHRT), HIV, (CCHRT), bronchitis, HIV and (HIV). After exclusion of studies that had an unknown outcome or no significant differences from placebo on the primary outcome, we conducted a search in PubMed and EMBASE for non-English original titles/articles and unpublished manuscripts. One of the authors (JL) extracted eligibility information into data analysis form, and all studies meeting eligibility criteria were eligible to be included. An analysis of cumulative risk (odds of a trial group achieving at least one of the primary outcome measures) was carried out. The primary outcome measures (defined as a measure of mortality and severe symptoms) were defined on the basis of the most recent assessment from the study in question and were compared with the outcome of interest (defined as a measure of survival) in the primary analysis. In the combined analysis, participants in the corticosteroids group demonstrated significantly greater relative efficacy in the primary outcome measure (HR, 0.86; 95% CI, 0.83-0.95; Figure A). In the combined analysis, COPD exacerbations in those receiving corticosteroids were significantly more likely to lead to mortality (1.3%; 95% CI, 1.1-2.0; Figure B).
